Circle of Care comes round for mental health
tal health care and addiction programs for Eastern Ontario got the green light. The Champlain Local Health Integration Network (LHIN) will launch a three-year pro- gram called Connecting the Circle of Care. The LHIN board approved the plan during
its May 29 meeting. The goal of the plan is to improve health and substance abuse addiction services for people throughout the Champlain LHIN. That includes the City of Ottawa, the Five Counties region, Renfrew and Lanark coun- ties, and part of Leeds-Grenville. The Circle of Care project is part of the LHIN’s strategic plan for the next three years. The plan includes increasing the capacity of
present mental health and addiction care and treatment services along with provid- ing more education and self-help training for clients, support programs for them and their families and their social worker sup- port network. Part of the goal of the plan is to reduce repeat visits to emergency centres and hos- pital readmission cases for clients dealing with mental health and addiction issues.

Health Links projects in the works
OTTAWA | A fast-track system for priority care cases may be part of the health ser- vice map for the Prescott-Russell region. The Champlain Local Health Integration Network (LHIN) is looking at Prescott-Rus- sell for one of the first projects in its pro- posed Health Links program for integrating primary, home and community care. The intent of Health Links is to organize health services for people identified as hav- ing the most complex needs. The program involves coordinating care plans for these individuals through a combination of con- sultations with primary, home, and commu-
nity care outlets, any needed specialists and hospital connections. The Champlain LHIN has presented the Ministry of Health and Long-Term Care with a readiness assessment for a Health Links program in Prescott-Russell. The LHIN also has another proposal for Health Link pro- gram that focuses on children with complex care needs in the Champlain region. Both assessment submissions are the first step to getting ministry approval for Health Links programs. The LHIN continues prepa- rations for Health Links readiness assess- ments for other areas within the region.
