• addressing immediate physical and safety needs, such as food, warm clothing and a place to stay • connecting people with housing, income support and health services; including making and accompanying them to appointments • linking to other support services, such as life skills training, personal health, household and financial management • acting as a landlord liaison when needed REACHING HOME SURREY COLLABORATIVE The Reaching Home Surrey Collaborative is a group of four local agencies. Lookout Society, Elizabeth Fry Society, Sources and Options are members of this partnership. This partnership uses a housing first focus to provide intensive case management to individuals who are chronically homeless. The housing first workers of the collaborative assist their clients in securing permanent housing then continue to work with them on building life skills and maintaining their housing for up to 2 years

Combined Outcomes of HOP and HPP 71 people were housed 29% of clients are referred to complimentary support services 5% of clients initiated a case plan

HOMELESSNESS PREVENTION PROGRAM (HPP) AND RENT SUPPLEMENT PROGRAM HPP provides people in identified at-risk groups facing homelessness with portable rent supplements to help them access rental housing in the private market.

The program focuses on the following groups: • youth transitioning out of foster care • women who have experienced violence or are at risk of violence • people leaving the correctional and hospital systems • individuals of Aboriginal descent

The HPP and Rent Supplement Program provided support to 238 individuals. The HPP program operates, in many instances, as an enhancement and partner with the existing Options’ Homeless Outreach Program.

Youth Services

The YS Department serves young people between the ages of 15 – 27 and is funded through The Ministry for Children and Families Development (MCFD), The City of Surrey, Services Canada, BC Housing, The Federation of BY Youth In Care, and philanthropic donors. Options has been
