HOW TO SUBMIT A WORK ORDER 1. Visit unk.edu/offices/reslife/ maintenance-request-form.php 2. Determine the appropriate department to contact: Facilities, Furniture, Laundry, Cable/Internet 3. If it is a Facilities issue select ‘UNK Facilities Work Request Site’ 4. Click ‘Submit Maintenance Request’ 5. Choose the Building you are entering a request for then proceed to next page >> 6. Insert your contact information and area (room number) that is being reported. 7. Provide detailed information in the ‘Request’ field as to what the request/situation is. 8. Select ‘Submit’ 9. You will receive an email confirming the submission of your request.
