Dear Colleagues, On behalf of the ACMS Board of Directors, I invite you to join me and your ACMS colleagues at the College’s 53 rd Annual Meeting in Seattle, Washington, May 6-9, 2021 at the Hyatt Regency Seattle. Invitation from the President

Dear Colleagues, On behalf of the Scientific Program Committee, I invite you to the 2021 American College of Mohs Surgery Annual Meeting in Seattle! This has been a difficult year for all of us. The pandemic has created profound difficulties across our membership and I want to personally thank you for your support of the College. I’m sure that many of you share Program Chair’s Message

The Scientific Program Committee has created a schedule of sessions that I am confident will expand and deepen your knowledge in the areas of Mohs surgery and reconstruction, patient outcomes, practice management, and much more. We missed our Annual Meeting in 2020. This has been an exceptionally challenging year for all of us, and we are really looking forward to the opportunity for interaction, either in person, virtually, or in both forms. Each year, the time we spend together during the Annual Meeting helps further develop not only our surgical skills and body of knowledge, but also renews our commitment to providing the very best patient care possible. This meeting is one of the few times we have each year to reconnect with colleagues, recharge our minds, and reengage with the important and timely issues facing our practices and our specialty. As we begin to prepare for the first-ever subspecialty board certification exams for micrographic dermatologic surgery, there has never been a more important time than now to showcase how and why Mohs surgery is the gold standard for skin cancer treatment. In addition to the educational content of the meeting, you will find opportunities to deconstruct the year that was 2020 at virtually. We have learned so much this year; some lessons much tougher than others. Throughout the meeting you will also be able to interact virtually with industry exhibitors and sponsors, learning how the resources, tools, services and information they provide may benefit your practice. My deepest thanks to Dr. Ian Maher, chair of the Scientific Program Committee, for his work leading the development of another outstanding and comprehensive meeting program. Thanks also to Dr. Jerry Brewer and the CME & Education Committee for their time and contributions. Dr. Sumaira Z. Aasi and the Board Certification Task Force must be acknowledged for their endless efforts to ensure that every ACMS member slated to sit for the ABD MDS exam will have the necessary content matter easily accessible. Additionally, thanks are due to Mary Riordan and the entire ACMS staff for their tireless work planning the many details that make the Annual Meeting a seamless and enjoyable event. my mix of excitement and trepidation when thinking ahead to the certifying exam on October 4 th . We have dedicated significant time within the program to board review and are confident that this will provide all attendees with a solid foundation on which to build their board preparation. As always we’ve carefully constructed the program to help you continue to deliver the best quality care, that ‘Mohs College Difference’, to your patients. Here are a few of the sessions I am particularly excited about: • Richard Anderson is a former CEO at Northwest and Delta Airlines as well as a former United Healthcare executive. He will discuss his career journey and how he used the development of key performance indicators to drive quality in the airline industry. With his healthcare payor experience he will give us insight into what ACOs and APMs may be looking for from skin cancer care. • I am excited to welcome two of my colleagues from the University of Minnesota, Dr. Samir Khariwala, Director of Head and Neck Surgery, and Dr. Christopher Wilke, Assistant Professor of Radiation Oncology. Dr. Hilda Stambuk, Director of Head and Neck Radiology at Memorial Sloan Kettering will also lend her expertise to the panel. I look forward to our guests sharing their insights on a collection of high risk tumors during the Tumor Board session. • Dr. Jason Newman of the University of Pennsylvania will give an ENT surgeon’s perspective on facial nerve anatomy. • The biggest thank you to Dr. Sumaira Aasi for leading the board review sessions at the Annual Meeting, and for editing the wonderful board review text which we are all familiarizing ourselves with! • Get your Kahoot! app ready because we’ve got a Trivia Night event led by Drs. David Carr and William Higgins! Thank you to Dr. Glenn Goldman, the Scientific Program Committee, CME & Education Committee, staff, and others who have contributed so much of their experience and effort toward plan this meeting. Those interested in attending the 2021 Annual Meeting may register by visiting www.mohscollege.org/annualmeeting and clicking on the Registration/Hotel tab, or use the tear-out form at the back of this program. Registration Confirmation Emails Registrants who mail or fax their registration will receive confirmation via email approximately two to three weeks after their registration and payment is received at the ACMS Office. Those registering for the Annual Meeting online will receive a confirmation almost immediately via email. Please print the confirmation and bring it with you to the meeting to help facilitate your registration process. Registration Pre-registration must be received by the ACMS Office on or before March 23, 2021. The office will not accept any registrations after April 12. Assuming that we are able to hold an in person meeting, the ACMS will make every effort to accommodate all members that wish

to attend the 2021 Annual Meeting in person. However, substantial attendance restrictions are likely. To that end, a lottery system will be implemented to allocate in person access. Additional information will be made available on the Annual Meeting registration site and as the situation evolves. The Registration Desk will be open at the Hyatt Regency Seattle on the following days and times (subject to change) :

Digital Diagnostic Quality Control Exam The Diagnostic Quality Control Exam is entirely online. This allows for more flexibility to review the slides, and allows completion of the exam before, during, and after the meeting. Instructions for accessing the digital DQC exam will be sent via the email address used for registration prior to the Annual Meeting. Access to the Diagnostic Quality Control Exam is $50 for ACMS members and free to Fellows- in-training. Only ACMS members who register for the exam and complete it online will receive a certificate. Certificates are mailed approximately 4-6 weeks after the meeting. CME credit is not offered for taking the digital Diagnostic Quality Control Exam. Participants must supply their own laptop or use a computer in the Business Center to take the exam on-site.

CME Information and Learning Objectives Accreditation Statement This activity has been planned and implemented in accordance with the Essential Areas and Policies of the Accreditation Council for Continuing Medical Education (ACCME) through the sponsorship of the American College of Mohs Surgery (ACMS). The ACMS is accredited by the ACCME to provide continuing medical education for physicians. Credit Designation Statement The ACMS designates this live activity for a maximum of 38.50 AMA PRA Category 1 Credit(s) TM . Physicians should claim only the credit commensurate with the extent of their participation in the activity. Physician Assistant Credit The American Academy of Physician Assistants (AAPA) accepts AMA PRA Category 1 Credit(s) TM from organizations accredited by the ACCME. Physician Assistants attending the Annual Meeting can submit certificates or transcripts showing how many physician CME credits were offered for an activity to the AAPA and get them “converted” to PA CME credit. The AAPA also grants and counts AMA PRA Category 1 Credit(s) TM , but those are specifically for PAs and have to come from a provider accredited by the AAPA. Both groups label their credits Category 1 CME, but the labels, though they read the same, refer to different evaluations. Learning Objectives Upon completion of the Annual Meeting, participants will be able to describe the latest advances in the treatment of skin cancer, discuss recent research findings in the area of Mohs micrographic surgery and cutaneous oncology, and explain new techniques in reconstruction that promote optimal surgical outcomes. Specific learning objectives, upon completion of the ACMS Annual Meeting, include: • Design and execute Mohs stages and routine and advanced reconstructions of the face, hands, nails, feet and genitalia; • Describe current recommendations for diagnosis and treatment of melanoma, and Merkel cell cancer; • Recall the benefits and techniques involved in utilizing immunohistochemistry in the treatment of melanoma and non- melanoma skin cancers as well as the indications and limitations of the same; • Refine reconstruction techniques to improve scars post Mohs micrographic surgery; • Identify anatomic landmarks and integrate knowledge of those landmarks into the practice of Mohs micrographic surgery; • Recognize potential errors in frozen section examination of skin cancers and develop ways to minimize those errors; • Explain proper billing and coding practices for Mohs and reconstructive surgery; • Recognize indications for multidisciplinary collaboration with medical and surgical colleagues; • Identify elements of the preoperative history that require management preoperatively; • Review the most recent literature regarding dermatologic surgery and cutaneous oncology; • Describe recent developments in the management of skin cancer in organ transplant recipients and other immunosuppressed populations; • Recall staging for cutaneous cancers including SCC, Melanoma, and Merkel cell carcinoma.

From 2014 until now, he is responsible for the annual basic surgery workshop of the European Academy of Dermatology and Venerology (EADV). Jason G. Newman, MD

Mr. Anderson attended Texas Tech University and the University of Houston, graduating from the latter in 1977 with a degree in political science. He earned a law degree in 1982 from the South Texas College of Law before working as a prosecutor for the district attorney office of Harris County, Texas. Mr. Anderson is a business executive whose career has included aviation industry tenures

Dr. Newman received his medical degree from Thomas Jefferson University Medical School in 1997 and completed residencies in

as legal counsel with Continental Airlines, and as CEO of Northwest Airlines and Delta Air Lines. He also served as Executive Vice President of United Healthcare from 2004 to 2007, and his most recent position as President, CEO, and senior advisor of Amtrak from 2017 to 2020. Samir S. Khariwala, MD, MS, FACS

Otolaryngology at Manhattan Eye, Ear & Throat Hospital, New York, NY, (1998-1999), New York Presbyterian Hospital of Columbia and Cornell, and Memorial Sloan Kettering Cancer Center (1999-2002). Following residency, Dr. Newman completed his fellowship in Head and Neck Surgery/Oncology and Microvascular Surgery at the University of Pennsylvania Perelman School of Medicine in 2005. Dr. Newman is board certified in Otorhinolaryngology and holds membership in the American Academy of Otolaryngology. His areas of expertise include head and neck mucosal and cutaneous cancer surgery, robotic surgery, anterior cranial base surgery and complex thyroid surgery with special interest in minimally invasive approaches, robotics surgery, and endoscopic cranial base surgery. Dr. Newman also is actively involved in research focused on head and neck cancer clinical trials, genetic signatures for head and neck Cancer, and quality of life for head and neck cancer and cranial base patients. As the Director of the Cancer Service Line of the Abramson Cancer Center at Pennsylvania Hospital, Dr. Newman has helped to create a disease-based team approach to cancer care delivery and research. This has helped to strengthen the delivery of cancer care, improve patient outcomes, create leadership pathways, increase patient volumes, increase access to clinical trials, and minimize variations in the delivery of care.

Dr. Khariwala is Professor and Vice Chair in the Department of Otolaryngology-Head and Neck Surgery at the University of Minnesota. He completed residency training at the Cleveland Clinic followed by a fellowship at the University of Michigan. His expertise is in the surgical management of benign and malignant tumors of the skin, mucosa and salivary/endocrine glands of the head and neck in addition to

microvascular reconstruction. Dr. Khariwala’s National Institutes of Health-funded research is focused on tobacco-related carcinogenesis in head and neck cancer. J. Marcus Muche, MD, PhD Dr. Muche studied medicine at the Humboldt University Berlin/Germany and at the Rijksuniversiteit Leiden/The Netherlands.

Afterwards, he specialized in dermatology and allergology at the Charité Berlin. In addition, he extensively practiced dermatosurgery.

In 1996, he acquired his MD at the Charité Berlin with research on molecular genetic techniques in the diagnostics of skin cancer and in 2010 he acquired his PhD at the University of Leiden with the application of those techniques in cutaneous lymphoma. Until 2003, he worked as dermatologist, allergologist and head of the molecular genetic laboratory of the department of dermatology at the Charité. From 2003 until 2012, he was dermatologist at the Westfries Gasthuis Hoorn. In 2012, he founded Mohs Klinieken Hoorn and in 2014 he became co-chairman of Mohs Klinieken.

Hilda E. Stambuk, MD

Christopher Wilke, MD, PhD

Dr. Stambuk is a diagnostic radiologist at Memorial Sloan Kettering Cancer Center with sub-specialty training in neuroradiology, including imaging of brain, spine, and head and neck disease. Her practice involves the interpretation of CT and MRI scans, and she has a particular interest in head and neck oncology. As a member of the Head and Neck Oncology Disease Management Team, Dr.

Dr. Wilke is the Assistant Professor in the Department of Radiation Oncology and the University of Minnesota Medical School and specializes in treating tumors of the brain, head

and neck and skin. He is a graduate of the University of Minnesota’s medical scientist training program where he stayed to complete his residency training before pursuing an advanced radiation oncology fellowship at MD Anderson Cancer Center. He maintains an active research focus in the fields of cancer imaging and computational tumor modeling.

Stambuk interacts with surgeons, medical oncologists, radiation oncologists, and other professionals in therapeutic decision making for patients with head and neck cancer. She also has particular expertise in image-guided biopsy of spine and head and neck tumors. Dr. Stambuk’s research interests include investigation of functional imaging (PET scans, functional MRI, and Dynamic Contrast Enhanced MRI) for the diagnosis, treatment, and surveillance of neurologic and head and neck disease.

Hand & Nail Workshop Wednesday, May 5 1:00 – 4:00 pm (3 00 Credit Hours) Location: University of Washington Institute for Simulation in Healthcare (WISH); Shuttle service for participants will be provided. $375.00 registration fee; for ACMS Members only Space is limited to 32 registrants and granted on a first-come, first- served basis, with 2 participants per cadaveric hand. Directed by Dr. Thomas Knackstedt, this workshop is geared toward all nail surgeons, from beginner and intermediate to advanced levels. It offers small group, hands-on experience practicing a variety of nail surgery techniques. Participants will be instructed on: • How to avulse the nail in several fashions (partial and complete) • Perform multiple different nail procedures (including punch, shave, and longitudinal techniques) • Treat nail melanoma with en bloc excision of all nail tissues and perform nail flaps (for those with more advanced background) Approaches to patient assessment, including surgical planning, equipment/ suture selection, and anatomical considerations to optimize outcomes will be covered. Anatomy as it relates to safe surgical technique will be stressed. Fresh frozen cadaveric hands that allow for realistic hands-on practice will be utilized.
