Hygiene and Disinfection

Dental staff and patients are continually exposed to a wide range of pathogenic micro- organisms – including bacteria, viruses and yeast. It is therefore vitally important to ensure a complete infection control range is utilised to minimise this ever-present threat within the dental surgery. It is equally as crucial that the products are efficacious against a broad spectrum of pathogens, tested to the latest microbiological standards and are effective in a quick but practical contact time for a given application. The Bossklein infection control range excels in each of these areas to provide the user and patient with complete peace of mind.

Bacteria It is essential that Bacteria is removed, due to their ability to rapidly reproduce through cell division. For example, under favourable conditions E.Coli can divide every 20 minutes – which means one bacterium can generate 1,048,576 bacteria in just 6 hours. The Bossklein infection control products are effective against a wide range of bacteria in a matter of minutes. This includes gram positive, gram negative and antibiotic resistant bacteria such as Staphylococcus aureus, E.Coli, MRSA and VRE*.

Viruses Bossklein infection control products are effective against viruses transmitted by blood (Hepatitis B, Hepatitis C and HIV) also viruses transmitted by direct or indirect contact such as Coronavirus or Influenza virus*. Viruses spread easily in high traffic areas, clinical areas and where the likelihood of contact with blood is high. Unlike bacteria – viruses rely upon living cells to replicate their viral material surviving for long periods of time. For example Hepatitis C can exist on surfaces for several weeks.

Yeast All Bossklein infection control products have been proven to be yeasticidal and tested for effectiveness specifically against Candida albicans. This should give both patients and staff complete confidence in all round infection control by Bossklein. Yeasts can lead to serious infections especially for those with comprised immune systems.

Efficacy vs Compatibility

Achieving a balance between microbial efficacy and material compatibility is crucial to ensure effective infection control without compromising the integrity of dental instruments, surfaces and equipment. Formulations and methods must be powerful enough to eliminate harmful pathogens, yet gentle enough to prevent corrosion, degradation, or weakening of delicate dental instruments, surfaces and restorative materials or harm to the users. Bossklein infection control products have been carefully designed to remove contamination and eliminate commonly found pathogens without damaging materials regularly used within the dental industry.

Transfer of Micro-organisms

The dental surgery is classed as a high-risk environment for the potential transfer of pathogens. Transmission can occur via multiple routes and without strict infection control procedures micro-organisms can rapidly transfer, increasing the risk of cross-infection between patients and staff. Patients and Staff Micro organisms have the potential to be easily transferred between directly patients and staff. Due to the close contact and relatively high traffic of a dental practise the risks are high. Indirect Contact Handling contaminated instruments, surfaces, or objects without proper sterilisation can spread micro-organisms. A comprehensive infection control procedure is vital to preventing transmission.

Droplets Coughing, sneezing or even talking can expel respiratory droplets. These can reach dentists, dental assistants and patients. Correct PPE can help prevent this alongside regular infection control. Airborne High-speed handpieces, ultrasonic scalers, and air-water syringes all generate airborne aerosols which can contain saliva and blood. These can linger in the air and settle on surfaces, potentially infecting staff or the next patient.

Importance of EN Testing

All Bossklein infection control products are tested to EN (European Norm) standards up to and including phase II steps I & II. EN testing is the gold standard due to its rigorous, real-world testing conditions and regulatory requirements. It is the most comprehensive and recognised method for evaluating disinfectants, particularly in dental / medical settings. EN standards are mandatory for disinfectants used on medical devices and sold within the European Union and the UK. They also align with biocidal product regulations, ensuring complete safety and effectiveness.

Three Stages of Infection Control

Cleaning Cleaning is a critical initial phase of the infection control process. The removal of organic matter including blood, saliva, and micro-organisms from surfaces or instruments is paramount to the success of later stages. Cleaning does reduce the number of micro-organisms but does not remove them all. It takes place with the aid of water, detergents and/or manual scrubbing or ultrasonic cleaning. If debris is left to remain it can effectively shield the surface from disinfection and sterilisation from taking place.

Disinfection Disinfection refers to the use of chemical agents (or heat) to reduce or eliminate the majority of pathogenic micro-organisms on surfaces or instruments. Active disinfectants include alcohol, chlorine or quaternary ammonium compounds (QACS). Regular disinfection is vital for high touch surfaces around the surgery like door handles and the dental chair. It is mandatory that these must be disinfected after each patient visit to avoid the potential transfer of diseases.

Sterilisation Sterilisation is the final stage in the infection control process. Ensuring complete destruction of all microbial life and preventing any risk of infection transmission This eliminates all micro- organisms including bacteria, yeast, viruses and spores. Sterilisation is achieved using heat and/or pressure in devices such as an autoclave or washer disinfector. All instruments must be autoclaved after cleaning and before reuse. Instruments are then placed inside sealed sterilisation pouches to ensure recontamination does not take place.

Skin & Hand Care

Thorough and complete cleaning and disinfection of special area items is absolutely necessary. These include aspirator/suction equipment and dental impressions. Bossklein Special Area products have been specifically developed to be used for these purposes. All inner surfaces of suction equipment are recommended for daily disinfection and cleaning with a product with no or low foaming properties

Hands have been proven to be the number one transmitter of pathogens in everyday life. It is considered crucial that hands are disinfected regularly. By using Bossklein Skin and Hand Care disinfectant products you reduce the rate of infections by a significant amount. With close contact between patients, dentists and nurses, cross-contamination is extremely high. To help combat this is regular and comprehensive hand disinfection must take place. The molecular properties of the Bossklein B70gel Hand Disinfectant and the inclusion of aloe vera allows for the preservation of the epidermis of the skin, whilst protecting against a wide range of organisms.

The demands of the modern dental surgery insist instruments are decontaminated, disinfected quickly and efficiently, anything less than this can result in lost hours repeating processes and can increase the risk of cross infection. Our Instrument Care products get to work quickly with their short contact times and take advantage of low dilution/ concentration levels to ensure time spent processing is minimal. Bossklein tests products against a variety of different materials to ensure maximum compatibility which is important of any instrument care product.

Dental, like other clinical environments including

medical, must adhere to strict requirements in order to keep staff and patients safe. One of the biggest risks is from cross-contamination between patients and staff coming into contact with the same surface. In any clinic the disinfection of surfaces is vital to stopping the spread of disease and infection. This can be achieved with both alcohol free and alcohol based products, usually in the form of a wipe or spray. Instrument Care

Disinfecting the instruments is the penultimate stage in the process before final sterilisation.

Cleaning

Disinfection An instrument disinfectant should be powerful enough to kill pathogens in 60 minutes or less according to EN14885. The effectiveness of the disinfecting process can also be enhanced by an ultrasonic cleaner. Additionally, excellent material compatibility is a crucial feature to prevent damage to instruments. Alongside a comprehensive effective spectrum which includes Coronavirus and TB, Bossklein IDactiv Instrument Disinfectant has completed extensive material compatibility testing according to EN21530 to preserve instrument quality.

Lubrication Efficient lubrication of handpieces not only ensures proper functionality and performance of a vital piece of dental equipment, regular maintenance can eliminate expensive repairs. It is vital that the lubricant reaches and coats the correct internal bearings of the handpiece so a pressurised aerosol solution is recommended. Bossklein UNIoil Universal Handpiece Oil has been specifically designed for modern dental handpieces. Pairing with the additional universal nozzle attachment, it can be used with any type of dental handpiece.

According to the UK HTM 01-05 guidelines – effective cleaning is essential before progressing onto the disinfection stage. Blood and organic soil can inhibit the instrument disinfectant antimicrobial efficacy. This is due to the fact that disinfectants have reduced efficacy in soiled conditions. Secondly, blood and organic soil occupy the instrument surface area, preventing the disinfectant covering and attacking pathogens on the instrument. The Bossklein ULTRAbio Ultrasonic Cleaner, when used in an ultrasonic bath, is ideal for helping to ensure protein has been removed from instruments by utilising protein digesting enzymes.

Surface Care

Disinfection is essential for ensuring that surfaces do not transfer infectious pathogens to patients and staff

Comprehensive cleaning and disinfection Disinfecting surfaces within the dental surgery is probably the most common and frequently performed infection control task. Due to patient traffic and the constant contact with contaminated objects, all equipment, surfaces, chairs, fixtures and fittings need to be disinfected between each treatment session. The advantages of the Bossklein range of surface disinfectant wipes and sprays are the thorough cleaning action and the compatibility with a wide range of surfaces. All Bossklein surface disinfectant wipes and sprays are virucidal against all enveloped Viruses including Coronavirus.

Alcohol sensitive surfaces

Alcohol resistant surfaces

Alcohol sensitive surfaces are prevalent in the dental surgery, for instance - X-ray equipment, hand controls and dental chairs. Dental chairs are a key area where patients share contact, and therefore an ideal surface area for the transmission of pathogenic micro-organisms. Dental chairs are typically made of artificial leather, therefore require a more sensitive cleaning and disinfecting treatment compared to other surfaces. The Bossklein alcohol free V-WIPE ZERO and alcohol free SDactiv ZERO Surface Disinfectant Spray are virucidal against all enveloped viruses, including Coronavirus, and are ideal for use on all surfaces, including those sensitive to alcohol.

Alcohol resistant surfaces are exploited in many areas within the dental surgery, for instance operating lamp handles. These surfaces remain unaffected by high alcohol content – which allows a powerful antimicrobial action. Unlike many other alcohol based surface disinfectants – the Bossklein alcohol based V-WIPE and SDactiv Surface Disinfectants have undergone complete microbial testing to ensure efficacy against all enveloped Viruses including Coronavirus. The short contact times and quick drying, streak free finish are major advantages when rapid disinfection is required eg in-between patients.

Why choose wipes? Disinfection using wipes should always be preferred to spray delivered liquid disinfectants for one main reason; its safer - for both patients and staff. Sprayed disinfectants become airborne and therefore there is an increased risk of inhalation. Wiping has the added benefit of the physical action of mechanically wiping a surface which can help to loosen debris and ensure the disinfectant makes full contact with the surface. Bossklein V-WIPE and V-WIPE ZERO have been specially designed for use on non-invasive medical device surfaces within the dental surgery, these include but are not limited to; dental chairs, worktops, plastic equipment cases, assistant’s stools, etc.

Each tub is fully recyclable and is provided with a secure cap/lid which includes an airtight seal keeping the wipes fresh for their next use. Tubs are designed to be easily refilled using the refill packs which helps to reduce the amount of plastic consumed and their associated carbon footprint.

Refill packs of V-WIPE and V-WIPE ZERO are available, in both the CLASSIC and ULTRA wipe substrates (see below for more details). These high quality, fully printed pouches are constructed from a strong triple laminate structure. Each pouch is designed to be used inside a reusable dispenser tub. Disinfection is essential for ensuring that surfaces do not transfer infectious pathogens to patients and staff.

Alcohol based products like V-WIPE Classic and Ultra are ideal to use on all alcohol resistant surfaces around the surgery. Their quick drying and streak free formula is a powerful weapon in the fight against infection within clinical environments.

In order to reduce the risk of infection for patients it is important that cleaning and disinfection take place regularly

Complete disinfection Aspirator/Suction systems become contaminated with organic matter and debris during the course of treatment, this can easily build up over time if efficient care and decontamination does not take place. In order to reduce the risk of infection for patients it is important that cleaning and disinfection take place regularly. It is recommended that they are cleaned/disinfected on a daily basis. Dental impressions are easily contaminated with pathogenic organisms due to their very nature - they come in contact directly with the inside of patients mouth’s. Decontamination and disinfection is vital to avoid cross- contamination during application, transport, processing and protect the surface quality of the impression itself.

Impressions Ensuring that the impression material is disinfected is vital to prevent the transmission of pathogens to the dental technician. It is mandatory that dental impressions are disinfected correctly before being sent to the lab for processing. It is typically the norm that impression disinfectant products should disinfectant in 10 minutes or under but they must not affect the mould stability and shape. Bossklein IMPactiv P impression disinfectant powder is an economical disinfectant, an especially quick contact time of just 60 seconds which helps to reduce overall processing times.

Suction systems The cleanliness and disinfection of the suction system should take place frequently - at least once a day, usually after surgery ends. The choice of disinfecting product should be limited to chemicals which are compatible with the system. For instance, chlorine based disinfectants will cause corrosion of the system. Skin and Hands

Effective hand hygiene is vitally important to prevent the spread of pathogens and contamination of instruments

Complete disinfection Due to the requirement to constantly disinfect hands at key stages, it is important the product has a rapid contact time and it is also delicate on the skin. Bossklein B70gel hand disinfectant has a 30 second contact time and is EN14476 virus tested for virucidal activity against all enveloped Viruses including Coronavirus. The efficacy of B70gel is further validated by performing microbiological tests to EN1500 standard which demonstrates the product efficacy in working practice. Including both aloe vera and vitamin E with the exclusive B70gel formula ensures hands remain supple and smooth, even after repeated use.

Protection and care

Consistency is the key The potential dangers of bad hand disinfection practises are obvious; an increased infection risk from the unavoidable close contact between staff and patients. It is considered vital that staff members are fully compliant in the process of hand disinfection as this is considered the most important method of reducing cross-contamination and therefore infection. To avoid increased risks hand disinfection must become consistent, comprehensive and be repeated frequently to become second nature.

The skin serves as the main protective barrier against damage to internal organs. The skin barrier is located in the uppermost layer of the skin, protecting the underlying skin from the entry of irritants and allergens. The UK Health and Safety Executive notes that healthcare workers may develop dermatitis more often than the general population. The most common causes are working with wet hands, in contact with liquid soaps and cleaning materials. Disinfecting hands to prevent infection is extremely important, Bossklein B70gel contains no dye to ensure maximum compatibility with minimum risks.

Hygienic Hand Disinfection Disinfection should be completed by applying Bossklein B70gel directly to dry hands and rubbing together over a minimum of 30 seconds. It is vitally important to rub every area of the hands including the finger nails and wrists. See below for our simple 8 step guide:

1 Apply a small amount (about 3ml) of gel in a cupped hand and cover all surfaces

2 Rub hands palm to palm

3 Rub back of each handwith the palm of the other hand with fingers interlaced

4 Rub palm to palm with fingers interlaced
