 Strengthen safeguarding practices for both adults and children in primary care settings.

This will be achieved by:

 Standardising complaint, incident reporting and learning processes across our services and our NPC managed practices.  Implementing safety alerts and shared learning forums across our organisation  Enhancing collaboration with community, urgent care, and secondary care partners to support safe care transitions and service delivery.

2. Strategic objective: Clinical Effectiveness

We will focus on improving outcomes in primary care through proactive, preventative, and evidence-based approaches, particularly for patients with long-term conditions and complex needs.

We will:

 Strengthen the delivery of structured care for long-term conditions in our NPC managed practices (e.g. diabetes, COPD, cardiovascular disease).  Improve early diagnosis and screening uptake, particularly in underserved populations our NPC managed Practices.  Support the effective use of multidisciplinary teams (MDTs), including ARRS roles, in delivering care.

This will be achieved by:

 Using population health management (PHM) data to target interventions and monitor outcomes.  Expanding clinical audit programmes linked to clear improvement actions.  Supporting clinicians to consistently implement NICE guidance and local pathways.  Increasing access to clinical supervision, peer review, and protected learning time.
